PCRC staff and contractor's will enforce the HIPAA Privacy Rule, which protects the privacy of our client's identifiable health information; which protects our clients information being used to analyze the client's elgibility.
ABOUT YOUR VISIT
Preparation for your appointment
The enrollment process begins with the parent interview. This interview will consist of one of our care coordinators visithig the your home or the parent coming into the office. Enrollments are by appointment only for a home visit or a walk- in can be taking during business hours.
The enrollment process consist of various forms to make sure you are a elegible for the program. Please bring your picture idenitification, pregnancy statement and your insurance card to your appointment. These forms are mandated by the state of Wisconsin Prenatal Care and Child Care Coordination Program.
